U wave is thought to be due to
1. Purkinje fiber repolarization
2. M cell (between endo- and epicardium) repolarization
3. Mechanoelectrical coupling (ex. MVP or Septal Bounce)

Postitive U wave typical for hypoK LVH or Dig Tox
Negative U wave typical for ischemia
